Chronicity matters. A diagnosis of chronic kidney disease generally requires abnormal kidney function or damage markers to persist for at least three months — not just a single low reading, which could reflect a temporary issue like dehydration or a recent illness.
Urine testing fills in the other half of the picture. Alongside eGFR, your care team often checks a urine albumin-to-creatinine ratio (ACR), which detects protein leaking into your urine — a sign of kidney damage that eGFR alone can miss. Patients with a normal eGFR can still have elevated ACR, and vice versa.
Muscle mass can distort the number. Because eGFR is calculated from creatinine, very muscular patients or patients who’ve lost significant muscle mass can get a result that doesn’t accurately reflect their actual filtration rate. In these cases, your provider may order a cystatin C test — an alternative marker that isn’t affected by muscle mass — to double-check the picture.
